New CMS icon recognizes Maple Farm’s strong quality, staffing and regulatory performance
AKRON, Pa. — Maple Farm has earned the new High Performing Facility designation from the Centers for Medicare & Medicaid Services (CMS). The recognition is now displayed with a trophy icon on Medicare’s Care Compare website, helping families identify nursing homes that meet rigorous quality and regulatory standards.
Christie Kennedy, Nursing Home Administrator, says, “This recognition reflects the care, commitment, and attention our team brings to residents every day. We are grateful for the continued support of our residents, families and community.”
CMS introduced the High Performing Facility icon as part of its nationwide Risk-Based Survey program. The icon became visible on Care Compare beginning October 8, 2026. The designation goes beyond a Five-Star overall rating by considering additional measures of staffing, survey performance, regulatory compliance and data accuracy.
To qualify, a nursing home must meet CMS criteria that include:
CMS evaluates eligibility quarterly. Facilities that continue to qualify may receive a more focused annual recertification survey that generally requires fewer surveyors and less time on-site. CMS and state survey agencies may still conduct a full survey when concerns about resident health or safety warrant one.
Facilities do not apply for the designation. CMS awards the icon based on its data, and the icon remains visible only as long as a facility continues to meet the program’s requirements.
